Hip dysplasia
The hip joint is a “ball and socket” joint. The head of the femur (thighbone) is the ball that fits into the acetabulum (socket) in the pelvis. Hip dysplasia deformity is an underdeveloped shallow socket that does not accommodate the head of the femur, which leads to an improperly aligned hip joint. This imbalance creates abnormal joint forces that adds excessive pressure on the joint surfaces causing it to wear out more quickly. This rapid wear and tear leads to pain, instability and eventual arthritis. Abnormal function also leads to soft tissue injuries like hip labral tears, pinched nerves, and an increased risk of joint dislocation.
Hip dysplasia is a developmental deformity of the hip that is often found in young babies and children, though sometimes it is not discovered until adolescence or young adulthood. In adolescents and young adults, hip dysplasia can cause significant pain and dysfunction and is a leading cause of early onset hip arthritis.
Hip dysplasia is diagnosed with plain x-rays as well as an MRI and CT scan to detect joint damage and reveal the true shape of the hip ball and socket.
A periacetabular osteotomy (PAO) surgery is a realignment surgery designed to add depth to the hip socket thereby improving joint mechanics and reducing wear and tear. This decrease in wear can enhances stability and postpones the development of osteoarthritis and subsequent need for hip replacement.
Given that near 100% of hips with dysplasia have concomitant intraarticular injuries, like hip labral tears, specialized centers around the world have developed techniques to perform same-day combined hip arthroscopy + PAO procedures.
